Book in advance with a licensed operator and the question stops being a question.

The short answer: your choice decides

Punta Cana is anchored by a modern international airport and built around tourism. The vast majority of visitors move between the terminal and their hotel without incident.

What varies is not the road — it is the operator. A regulated company works inside a legal framework of licensing, vehicle inspection and insurance requirements. An informal driver works outside it, and you have no way to verify anything in the thirty seconds you have to decide.
