There is no version of the map where La Romana is the closer airport to Secrets Cap Cana. Punta Cana sits much nearer the resort, and any transfer from PUJ to Cap Cana is a shorter piece of road than the equivalent run from La Romana. What the longer drive actually involves

Once you've decided LRM makes sense for a fare or schedule reason, the drive itself is straightforward: east from the airport, through La Romana's own outskirts and past Casa de Campo, before joining the roads that lead to Cap Cana's Coral highway and Boulevard Turístico del Este approach. It is a longer run than the Punta Cana equivalent, and we won't describe it as anything else.

We monitor the inbound flight the same way we would for any airport, so a delay shifts the pickup with it. The only real difference from a Punta Cana booking is the length of the drive that follows, not the way the booking itself is handled.

Some groups end up split between La Romana and Punta Cana simply because they booked flights separately — one traveller found a better La Romana fare, another booked a direct Punta Cana flight months earlier. That's a manageable situation, not an unusual one, and it doesn't require picking a single 'correct' airport for the whole party.
